Hands On Deck General Liability Waiver
Version 3 · Effective 2026-09-02
HANDS ON DECK GENERAL LIABILITY WAIVER
Who this waiver covers
This waiver applies to any person taking part in a Hands On Deck (HOD) class, program, activity, event, volunteer assignment, shop visit, At Home activity, online activity, water activity, or other HOD offering. The participant is the person taking part. The signer is the person legally signing. A participant who is 18 or older signs for themself. For a participant under 18, a parent or legal guardian signs for the minor.
Acknowledgment and assumption of risk
HOD activities may involve tools, machinery, sharp edges, heat, adhesives, finishes, materials, watercraft, facilities, or project kits. Risks may include cuts, scrapes, punctures, burns, falls, allergic reactions, property damage, serious injury, death, or other harm. HOD takes care to provide instruction and a safe environment, but some risks cannot be eliminated. I voluntarily choose participation and accept the known and unknown risks for myself or for the minor participant.
Safety and conduct
The participant agrees to follow HOD instructions and safety rules, use required safety equipment, behave respectfully, and stop any activity when directed. HOD may limit or end participation when needed for safety.
Release of liability
In consideration for being allowed to participate, I release Hands On Deck Inc. and its board members, directors, officers, employees, instructors, volunteers, and agents from claims for personal injury, property damage, or other loss arising from participation, except claims arising from gross negligence or intentional misconduct.
Emergency medical care
If an injury or illness occurs, I authorize HOD to obtain emergency assistance or medical care when reasonably necessary. I understand that I am responsible for resulting medical costs for myself or for the minor participant.
Agreement
I have read and understand this waiver. I sign it freely and voluntarily for myself or, if the participant is under 18, as the participant's parent or legal guardian. My electronic signature is intended to have the same effect as a handwritten signature.
